One of the most prevalent diseases worldwide, challenging the community is diabetes mellitus (DM). Diabetes, a chronic disease associated with an imbalance in insulin and the associated response of the body to the said conditions, common of which is diabetic neuropathy. DM neuropathy, in the setting of also increased plantar foot pressures, had long been recognized as high risk factors for the development of pedal ulceration. It is a type of nerve damage affecting 60 to 70 percent of people with type 1 and type 2 forms. Currently, the risk of development of pedal ulceration for patients diagnosed with diabetes is placed at 15%. For this study, plan is to investigate the relationship between in-shoe plantar foot pressure and the activity of the anterior tibialis muscle. This muscle is responsible for dorsiflexion and inverting the foot and performs an eccentric (lengthening) contraction, which prevents foot slap after the heel-strike phase of walking. The proposed solution is a body worn device that consists of two main subsystems to record peak plantar pressures in diabetic patients and determine the amount of stimulation needed to fire the anterior tibialis to signal for gait alteration and plantar pressure reduction in real time. The system is a light and portable embedded system that provides bi-phasic electric square pulses to the muscle through electrodes attached to the skin. The results show that the system stimulating the anterior tibialis muscle, when the sensor pressure readings are higher than the threshold, leads to plantar pressure reduction and gait alteration.
