Lifestyle interventions for the prevention of diabetes after pregnancy, Linda, Brasil - main features and initial resultsBackground Women with gestational diabetes are at great risk of developing diabetes and thus in great need to engage in diabetes prevention programs. However, women at post-partum are usually very concerned with their new motherhood responsibilities and show generally low interest in undertaking additional activities. We have developed a diabetes prevention program tailored to women at post-partum and initiated a randomized controlled trial to test its efficacy when offered in a more intensive (phone coaching and written materials) versus a less intensive approach (only written materials). Aim To describe main features of the program and present process outcomes of the initial group who completed one year of follow up during the trial. MethodsLINDA-Brasil (Life Style Interventions for Diabetes After Pregnancy) is a randomized controlled trial among women with high risk gestational diabetes. High risk of diabetes was defined by the use of insulin during pregnancy and/or by presenting prediabetes at post-partum based on fasting or 2h PG. The intervention components are breastfeeding and healthy life style changes. Healthy eating emphasizes reducing high energy foods, with a focus on ultraprocessed foods, and increasing fruits and vegetables, with emphasis in less processed preparations. Increase in physical activity is stimulated gradually, with the aim of reaching better physical fitness. Exercises are adapted to the in a gradual intensity, at home, including an interval training program. Support for maintaining breast feeding is given. Phone coaching consists of 14 core sessions. Main outcome is diabetes, assessed yearly, with results blinded to the staff involved with participants.ResultsWe have actively randomized 119 women. Mean age is 31.1 years, 92% being either overweight or obese. Women are in general of low social status, most of them not reaching university. A total of 50 women have completed one year of follow up. Among them, 24 were randomized to more intensive intervention (phone coaching): 2 did not engage in the program, 1 became pregnant, the remaining 21 (88%) adhered to the program, although only 15 (62%) completed all core sessions. Compared to those who received less intensive intervention, the more intensive group lost 3.4 kg (vs gained 1.1 kg ), decreased 6.1cm of waist circumference (vs 1.5 cm) and stopped breast feeding in 8.3% (vs 21.1% ). DiscussionDespite difficulties, the phone coaching was generally well received by women with high risk recent gestational diabetes. Initial analyses of process outcomes suggest that simple ways of delivering support for life style changes may produce relevant outcomes.
